Range Rental Info Police Dept Use only

In order to request the use and rental of a Range at Holbrook Sportsmen's Club you will need to provide some information with your request.

1.  We will need the Date Requested.   MM/DD/YYYY   _______________________________________________You will be notified if accepted.

2.  A copy of your insurance binder naming Holbrook Sportsmen's Club on the binder.

3.  A Completed Holbrook Sportsmens Lease Agreement return to the Club President.  Download PDF here or the link below.  Can be filled out by hand or via Adobe Acrobat and emailed.

4.  A Contact person who will serve as the designated Range Officer for the rental period.

5.  Submit request 60 days prior if possible.
